• I have met with my Ministry Placement Supervisor, and they have completed the Primary Ministry Placement Review Form for Term 2. You can find a copy here.
• Upload a copy of the signed Primary Ministry Placement Review Form for Term 2.
• Look at what your Ministry Supervisor as marked as requires work or developing on the Primary Ministry Placement Review form. For each point, ask your Ministry Supervisor how you could improve in this area before the next review and write these suggestions down below.

IMPORTANT: It is important that in the Primary Ministry Placement Review Form that you are consistent in all the points in order to achieve competency.

• Upload a copy of Documenting CMT Hours Spreadsheet. This needs to include all the hours that you have completed for your Primary Ministry Placement, Camp Ministry Placement, Church Community and Discipling for Term 2.  
• Have you encountered a person that required you to access wellbeing resources? If not, you will need to make up a potential scenario. Explain the situation.
• Describe what you did or could have done. How did you or how would you complete a report about this issue according to Ministry Placement’s procedure?
• What support and resources are available to you to deal with this issue? What two resources would you use to help this person?
• How did or would you supply information to the group about to help them access support?
• How did or would you encourage the cooperative use of this resource?
